How to Enroll

1. Check 'Do You Qualify' to make sure you follow eligibility requirements

2. Call or visit your local intake agent to schedule an enrollment appointment (see link below)

3. Print Checklist and remember to bring the following:

Proof of Income (bring copies of all the documents that apply to your households   income situation)

• Copies of pay check stubs (most recent 60-90 days)
• Proof of general assistance, food stamps, alimony, or child support
• Proof of unemployment benefits
• Proof of social security benefits
• If paid in cash or lacking sufficient paystubs, please provide a letter from employer on letterhead indicating typical hours, salary, gross income, etc

* While this list represents most of the documents Access DuPage would typically request to document income, in certain circumstances Access DuPage may request alternate means of demonstating financial means, including documentation of the financial resources of individuals providing support to you, recent tax documents, and/or documentation of assets such as retirement income, savings income, inheritance, etc.

Proof Of Address (Please provide ONE of the following)

• Utility Bill (must be current and issued within last 30 days)
• Drivers license or state id  (only if issued within the last 30 days)
• Current Lease or proof of current mortgage

If you live with someone else and have none of the documents listed above documenting address, please provide the following:

• Provide a letter from person who you live indicating that you are residing with them
• Have the person writing the letter provide one of the above proof of address documents in their name

Family members (Individuals who are applying for Access DuPage but cannot attend the application agent appointment in person)

• Each applicant will need to sign their own copy of a Release of Information form, as well as notification of Good Samarity Act Immunity Waiver
• These forms are available on this website and can be printed at your convenience if every member cannot make appointment. Applications received without a completed copy of the two listed forms will not be processed until the forms are received.
