Do You Qualify?

WHO IS ELIGIBLE FOR ACCESS DUPAGE?

To be eligible for Access DuPage, you must:

• Be a permanent DuPage County resident for at least 30 days ( this means that you live or intend to live in DuPage County year round)
• Be between the ages of 19 and 64
• Have a household income at or below 200% of the current federal poverty guidelines aspe.hhs.gov/poverty/09poverty.shtml 
• Not be eligible for health care programs such as (Medicaid, Medicare, employer sponsored insurance, student health services, COBRA, SSI, Family Care, Veterans Benefits, etc.)

Most persons in Access DuPage are enrolled for a one-year period. At the end of each period of eligibility, persons may re-enroll if they still meet the eligibility requirements.

All persons enrolled in Access DuPage receive an ID card. This card must be presented at all times by the enrollee to receive any medical services. Access DuPage enrollment does not guarantee that all medical costs will be covered.  Any services provided to patients but not written off or donated by participating providers are the responsibility of the patient.
